ACTIVITY:  Various Sacred Heart Youth Group and Confirmation activities and day trips throughout the 2024/2025 school year.
LOCATION:  Various locations in Fruita and Grand Junction
I voluntarily wish to participate in the above described activity.  As a condition of my being allowed to do so, I hereby, to the extent permitted by the law, release and discharge the Diocese of Pueblo, its constituent organizations, including but not limited to Sacred Heart, and their affiliates, officers, agents and employees, from all claims for personal injuries or property damage I may suffer as a result of my voluntary participation in the activity described above, including but not limited to any transportation to and from the event.  This release includes whether or not such injuries or damage are caused by the negligence (active or passive) of any of the entities or individuals named or described in this release.  

This Waiver and Release is intended to release and discharge in advance the promoters, sponsors, officials, leaders, the Diocese of Pueblo, the Bishop of Pueblo, a Corporation Sole, and Sacred Heart and their affiliates, officers, agents, and employees from all liability arising out of or connected in any way with my participation in the activity.

I agree to abide by the rules and regulation governing the above described activity and to obey any instructions given by the person or persons having supervision and control over the activity.
